Hospice Registered Nurse (RN) providing individualized nursing care that maximizes comfort and quality of life. Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans. Perform comprehensive assessments—physical, emotional, social, and spiritual—to guide care decisions. Educate and support patients and caregivers. Coordinate care across disciplines to ensure continuity and dignity. Document care clearly and accurately to reflect patient needs and progress. Participate in on‑call or admission rotations as needed. Engage in ongoing learning and professional development.
