*Job Summary*
Seeking an experienced RN Case Manager to develop, implement, coordinate, monitor, and evaluate case-management plans. The RN will work closely with patients, physicians, healthcare providers, staff, and payers to coordinate appropriate, effective, and cost-efficient care.
- *Requirements*
- BSN required; MSN preferred
- Minimum 3 years hospital clinical experience
- Minimum 5 years hospital case-management experience
- Current unrestricted Colorado RN license
- Current BLS
- Case Management certification preferred, or willingness to obtain within 2 years
- Knowledge of discharge planning, utilization management, and continuum of care
- Strong documentation, communication, coordination, and patient advocacy skills
- *Key Responsibilities*
- Develop and evaluate case-management plans
- Coordinate patient care with physicians and healthcare teams
- Collaborate with payers and review organizations
- Support appropriate utilization of healthcare resources
- Document case-management activities
- Educate physicians and staff regarding level-of-care and utilization issues
- Coordinate discharge planning and continuum-of-care services
- Participate in Case Management/Social Services meetings
Pay: $50.00 - $52.00 per hour Work Location: In person
