*Job Summary*

Seeking an experienced RN Case Manager to develop, implement, coordinate, monitor, and evaluate case-management plans. The RN will work closely with patients, physicians, healthcare providers, staff, and payers to coordinate appropriate, effective, and cost-efficient care.

• *Requirements*
• BSN required; MSN preferred
• Minimum 3 years hospital clinical experience
• Minimum 5 years hospital case-management experience
• Current unrestricted Colorado RN license
• Current BLS
• Case Management certification preferred, or willingness to obtain within 2 years
• Knowledge of discharge planning, utilization management, and continuum of care
• Strong documentation, communication, coordination, and patient advocacy skills
• *Key Responsibilities*
• Develop and evaluate case-management plans
• Coordinate patient care with physicians and healthcare teams
• Collaborate with payers and review organizations
• Support appropriate utilization of healthcare resources
• Document case-management activities
• Educate physicians and staff regarding level-of-care and utilization issues
• Coordinate discharge planning and continuum-of-care services
• Participate in Case Management/Social Services meetings

Pay: $50.00 - $52.00 per hour Work Location: In person
