RN Case Manager (Home Health - Office) FT

● Ensure the team is educated on the needs of individuals, families or communities, including assessment of individuals' home or work environments to identify potential health or safety problems. ● Inform physician of patient's condition as required. ● Observe nurses and visit patients to ensure proper nursing care. ● Professionally manage patient, staff, and doctor issues as needed. ● Act as a liaison between the doctor and staff. ● Direct and coordinate infection control programs, advising and consulting with specified personnel about necessary precautions. ● Recommend where appropriate drugs, medical devices or other forms of treatment, such as physical therapy, inhalation therapy, or related therapeutic procedures. ● Consult with institutions or associations regarding issues and concerns relevant to the practice and profession of nursing. ● Engage in training or research activities related to nursing. ● Perform other duties as assigned by management. POSITION QUALIFICATIONS: The RN shall be currently licensed by the LSBN without restrictions and have, at a minimum, one year of clinical experience as an RN. This requirement may be waived for an RN with one year's clinical experience as an LPN. SKILLS & ABILITIES Education: Registered Nursing Program Experience: At least one year of RN Home Health experience required, two years related experience preferred. Computer Skills - WellSky - Google Documents Certificates & Licenses - Valid Louisiana RN license without restrictions. WORK ENVIRONMENT Primary or Remote office and occasional patient home visits.
