Your Role as a Home Health LPN
- Provide skilled nursing care under the supervision of a Registered Nurse
- Perform patient assessments and assist with care planning
- Administer medications and treatments as ordered
- Educate patients and caregivers on disease management and care plans
- Monitor patient condition and report changes to the supervising RN
- Collaborate with physicians and interdisciplinary teams
