Full-Time | Monday–Friday | Day Shift
Heather Hill Health Care Center is seeking an experienced RN MDS Coordinator to join our clinical leadership team.
This is not an entry-level MDS position. We are looking for a knowledgeable Skilled Nursing professional who understands how accurate assessments, strong documentation, PDPM, Quality Measures, and interdisciplinary collaboration come together to drive both exceptional resident outcomes and strong facility performance .

What You'll Own
As our RN MDS Coordinator, you'll take an active leadership role in the facility's MDS, reimbursement, care planning, and quality processes.
Responsibilities include
- Manage the complete MDS/RAI process from assessment and coding through review and submission
- Ensure MDS assessments are completed accurately and within required timelines
- Drive accurate PDPM classification and reimbursement
- Review clinical documentation to ensure it supports MDS coding and resident acuity
- Coordinate with Nursing, Therapy, Social Services, Dietary, and other interdisciplinary departments
- Lead and participate in resident care planning
- Monitor CMS Quality Measures and identify opportunities for improvement
- Track and evaluate Case Mix Index (CMI) trends
- Support Managed Care authorizations, payer communication, and case management processes
- Educate nursing and interdisciplinary staff on documentation requirements
- Identify documentation gaps and proactively work with clinical teams to improve accuracy
- Support reimbursement reviews and audits
- Maintain compliance with CMS, state, federal, and facility requirements
- Participate in QAPI initiatives and ongoing survey readiness
What We're Looking For
Required
- Current Florida Registered Nurse (RN) license in good standing
- Minimum 2 years of MDS experience in a Skilled Nursing Facility
- Strong working knowledge of MDS 3.0 and the RAI process
- Strong understanding of PDPM
- Knowledge of CMS Quality Measures
- Understanding of reimbursement and Case Mix
- Strong clinical documentation and assessment skills
- Ability to manage multiple deadlines with exceptional attention to detail
- Strong communication and interdisciplinary leadership skills
Highly Preferred
- RAC-CT Certification
- PointClickCare (PCC) experience
- Skilled Nursing Case Management experience
- Managed Care and insurance authorization experience
- Experience with reimbursement audits
- Experience monitoring and improving CMI and Quality Measures
