Job Purpose
Provides nursing services in the patient’s home in accordance with agency standards and applicable laws and regulations. Ensures that patients receive the highest possible quality of care.
Key Responsibilities
Ensure quality and safe delivery of home health care services.
Implement current nursing practice to meet the needs of patients.
Fulfill additional performance responsibilities as assigned and as necessary to support patient care.
Promote effective written and verbal communication with patients, families, and healthcare teams.
Use equipment and supplies effectively and efficiently to provide optimal care.

Education
Completion of a state-approved practical nursing program

Experience
Experience in providing nursing services in a home care setting
Familiarity with applicable laws and regulations governing home health care
