What You'll Do

As a Home Health RN, you will provide skilled nursing care in patients' homes while coordinating with physicians and the interdisciplinary care team.

Responsibilities include

• Completing skilled nursing visits according to physician, nurse practitioner, or physician assistant orders

• Performing patient assessments and developing individualized care plans

• Educating patients and families on medications, disease management, and recovery goals

• Monitoring patient progress and communicating changes in condition to providers

• Documenting care accurately using an electronic medical record (EMR)

• Collaborating with therapists, aides, and care team members to achieve positive patient outcomes

• Participating in team meetings, patient care conferences, and quality improvement activities

• Participating in an on-call rotation

Qualifications

Required

• Current Tennessee Registered Nurse (RN) license

• Current CPR certification

• Valid driver's license

• Reliable transportation and current auto insurance

Preferred

• Previous Home Health RN experience

• Experience with OASIS documentation and Medicare home health requirements
