Havencrest Rehabilitation & Healthcare Center is seeking an experienced and motivated Registered Nurse Assessment Coordinator (RNAC) to join our team!
Schedule:
• Monday – Friday (Hybrid)
• 8:00 AM – 4:00 PM

Responsibilities
• Complete and coordinate resident MDS assessments accurately and within required timeframes.
• Evaluate residents' clinical status and identify changes that may impact reimbursement or care planning.
• Ensure compliance with Medicare, Medicaid, and all applicable state and federal regulations.
• Collaborate with the interdisciplinary team to support quality resident outcomes.
• Maintain accurate documentation and participate in care planning and reimbursement processes.
Qualifications
• Current Registered Nurse (RN) license
• Previous long-term care experience required
• Previous MDS/RNAC experience required
• Strong organizational, assessment, and communication skills
• Working knowledge of Medicare and Medicaid reimbursement guidelines
