*Job Summary* We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our team as a Field Nurse. In this vital role, you will provide comprehensive nursing care directly in patients' homes and community settings, ensuring high-quality, compassionate support for individuals with diverse healthcare needs. Your expertise will help promote health, safety, and independence for clients across a variety of environments, including long-term care facilities, assisted living communities, and private residences. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working independently in the field. *Responsibilities* * Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to determine individual care needs. * Administer medications via injections, IV infusions, and other routes while adhering to strict safety protocols and documentation standards. * Manage complex care plans involving dementia, Alzheimer's disease, memory care, and other chronic conditions with a focus on pain management and symptom control. * Provide specialized care such as catheterization, tube feeding, ventilator management, wound dressing changes, and infection control procedures. * Collaborate with physicians, case managers, and caregivers to develop and implement personalized care strategies that meet regulatory standards and best practices. * Document all patient interactions accurately within Electronic Health Record (EHR) systems like Epic or eClinicalWorks while maintaining strict confidentiality in accordance with HIPAA regulations. * Educate patients and their families on health maintenance, medication administration, safety protocols, and disease management to foster independence and well-being. *Experience* * Valid LVN license with current state licensure required; additional certifications such as Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) are highly preferred. * Proven experience working as a licensed vocational nurse in diverse settings including long-term care facilities, hospitals (including ICU or trauma centers), hospice or palliative care environments. * Demonstrated expertise in managing complex cases involving acute pain management, infectious diseases, geriatrics, dementia care, and emergency response situations. * Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks; experience with ICD-9/ICD-10 coding is advantageous. * Strong knowledge of physiology, anatomy, medication administration techniques including venipuncture and sterile procedures. * Experience working with individuals with disabilities or developmental challenges is desirable; familiarity with medical imaging or laboratory specimen collection is a plus. * Excellent communication skills for effective caregiver collaboration and patient education in home or community-based settings.
