Licensed Practical Nurse

• Perform patient intake, obtain and document vital signs, medical histories, medication lists, allergies, and chief complaints.
• Assist providers with examinations, procedures, treatments, and minor office-based interventions.
• Provide nursing care for patients with acute illnesses, injuries, chronic conditions, and preventive healthcare needs.
• Administer medications, injections, immunizations, and treatments as ordered and within the LPN’s scope of practice.
• Perform and assist with point-of-care testing, including laboratory testing, specimen collection, and diagnostic procedures as applicable.
• Prepare patients for procedures and provide appropriate post-procedure care and instructions.
• Monitor patients for changes in condition and promptly communicate significant findings to the provider.
• Perform wound care, dressing changes, ear irrigation, splinting, and other clinical services within scope and clinic protocols.
• Assist with management of chronic conditions, including routine follow-up, patient education, and monitoring.
• Provide patient education regarding medications, treatments, preventive care, follow-up appointments, and home-care instructions.
• Support telephone and electronic clinical triage according to established protocols and scope of practice.
• Review and document patient information accurately in the electronic health record (EHR).
• Assist with prescription refill requests, referrals, prior authorizations, and other clinical workflows as directed.
• Maintain exam rooms, supplies, equipment, and medication areas in accordance with infection-control and safety standards.
• Follow OSHA, HIPAA, infection prevention, medication safety, and other applicable regulatory requirements.
• Recognize and appropriately respond to urgent or emergent patient needs, including initiating appropriate emergency procedures when indicated.
• Promote a positive patient experience through respectful, professional, and culturally sensitive communication.
• Perform other duties as assigned to support efficient clinic operations.

Qualifications

• Current, unrestricted LPN/LVN license in the state of practice.
• Graduate of an accredited practical nursing program.
• Current BLS certification.
• Previous outpatient, primary care, urgent care, or ambulatory healthcare experience preferred.
• Knowledge of basic nursing principles, infection control, medication administration, and patient safety.
• Proficiency with electronic medical records and standard clinical technology preferred.

Work Environment

This position is based in an outpatient clinic serving both scheduled primary care patients and patients seeking same-day care for acute illnesses and injuries. The LPN should be comfortable working in a fast-paced environment where patient volume and clinical priorities may vary throughout the day.

Physical Requirements

The position may require prolonged periods of standing and walking, frequent sitting and computer use, and the ability to assist with patient mobility and perform routine clinical procedures. The LPN must be able to safely perform the essential functions of the position with or without reasonable accommodation.
