Licensed Practical Nurse

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate healthcare team. The ideal candidate will possess strong clinical skills, a passion for elder care, and experience working with individuals with memory impairments. As an LPN, you will play a vital role in delivering high-quality nursing care, supporting residents' health and well-being while ensuring compliance with regulatory standards.

*Duties*

• Provide direct patient care, including medication administration, minor first aid and vital signs monitoring.
• Assist residents with activities of daily living, ensuring comfort and dignity at all times.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Document resident health status accurately using electronic health records (EHR/EHR systems).
• Monitor residents for signs of acute or chronic health issues, including Alzheimer's and dementia-related conditions.
• Support infection control protocols and maintain sterile techniques during procedures such as catheterization and phlebotomy.
• Educate residents and their families on health management, medication use, and safety procedures.

*Skills*

• Valid licensure as a Licensed Practical Nurse (LPN) in accordance with state regulations.
• Experience working with geriatric patients, including memory care, Alzheimer's, and dementia populations.
• Proficiency in electronic health records (EHR) systems such as Epic or Cerner.
• Knowledge of medication administration protocols and IV infusion techniques.
• Strong assessment skills with the ability to recognize changes in residents’ conditions promptly.
• Familiarity with long-term care regulations, HIPAA compliance, and infection control policies.
• Excellent communication skills to effectively collaborate with team members, residents, and families.

Pay: $27.00 - $30.00 per hour
