About this role

We are seeking a dependable and experienced *Registered Nurse (RN)* to join our home health care team. The RN will be responsible for completing patient assessments, developing and updating plans of care, performing supervisory visits, and helping ensure that clients receive safe, high-quality care in their homes.

Responsibilities

  • Complete initial nursing assessments and Start of Care visits
  • Complete recertifications and Resumption of Care visits as needed
  • Develop and update individualized Plans of Care
  • Perform skilled nursing visits when required

  • Complete supervisory visits for home health aides
  • Monitor changes in clients' conditions and communicate concerns appropriately
  • Coordinate with physicians, case managers, caregivers, and agency staff
  • Ensure nursing documentation is accurate and completed timely
  • Assist with maintaining compliance with agency policies and applicable state and federal requirements

Qualifications

  • Current and unrestricted *Ohio Registered Nurse (RN) license*
  • Minimum of 1 year of nursing experience preferred
  • Home health experience preferred
  • Strong assessment and documentation skills
  • Reliable transportation and valid driver's license
  • Ability to travel to clients' homes within the service area
  • Dependable, professional, and able to work independently
