About this role

As a Home Health Registered Nurse (RN), you will provide compassionate and comprehensive nursing care to patients in their homes. Your role will involve assessing, planning, implementing, and evaluating patient care in collaboration with physicians, caregivers, and other healthcare professionals. You will be responsible for managing a diverse caseload of patients with various medical conditions, ensuring the highest standards of care are maintained. Schedule: PRN, Weekdays (M-F), 8:30AM - 4:30PM Primary Responsibilities and Duties: Conduct comprehensive patient assessments and develop individualized care plans based on patient needs and medical history. Administer medications and treatments as prescribed by physicians. Monitor and document patient progress, changes in condition, and response to treatments. Educate patients and their families on disease processes, medications, and home care techniques. Collaborate with physicians, therapists, and other healthcare professionals to coordinate and optimize patient care.

Provide wound care, catheter care, and other specialized nursing procedures as required. Perform health screenings and diagnostic tests, including taking vital signs and managing chronic conditions. Ensure compliance with healthcare regulations, protocols, and best practices. Maintain accurate and up to date patient records and documentation. Offer emotional support and guidance to patients and families throughout the care process. Perform all other duties assigned.

Requirements

  • Valid license as a Registered Nurse (RN) in both KS and MO
  • Some nursing experience in home health preferred but not required
  • Strong clinical skills and knowledge of nursing principles, procedures, and best practices in long-term care
  • Excellent communication and interpersonal skills
  • Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment
  • Understanding of regulatory requirements governing nursing practices and home health agencies
  • Ability to build rapport with patients through a compassionate and caring demeanor
  • Ability to lift and transfer patients up to 50lbs
