Role Description The Home Health RN for Irving and surrounding areas is a full-time role based in Dallas, TX. The RN will perform in-home skilled nursing visits, including assessments, wound care, medication management, and monitoring of chronic and acute conditions. Day-to-day responsibilities include developing and updating individualized care plans, coordinating with physicians and interdisciplinary team members, and educating patients and caregivers on disease processes, treatments, and self-care.
The RN will document clinical findings accurately and timely, ensure regulatory compliance, and contribute to high-quality patient outcomes. The role also involves participating in case conferences, supporting seamless transitions of care, and promoting patient safety and comfort in the home environment.
Qualifications
- Strong clinical foundation in Nursing with the ability to perform comprehensive assessments and deliver evidence-based care.
- Experience in Home Care and patient care, including providing services in residential settings and collaborating with families and caregivers.
- Proficiency in Wound Care, including evaluation, treatment, and ongoing management of simple and complex wounds.
- Active, unrestricted Registered Nurse (RN) license in the state of Texas.
- Prior home health or hospice experience preferred; familiarity with CMS regulations and OASIS documentation is an advantage.
- Excellent communication, interpersonal, and critical thinking skills, with the ability to work independently and manage a mobile caseload.
- Valid driver’s license, reliable transportation, and willingness to travel within Irving and surrounding areas.
- Basic life support (BLS) certification; additional specialty certifications in wound care or geriatrics are beneficial.
Pay: $82,000.00 - $90,000.00 per year
