About this role

We are seeking a Registered Nurse (RN) to conduct in-home client assessments and support the delivery of quality home care services. The RN will assess client needs, develop and update Plans of Care, monitor health status, coordinate services, and complete required clinical documentation.

Responsibilities

  • Conduct initial and follow-up home assessments.
  • Evaluate client health, functional needs, medications, and home environment.
  • Develop and update individualized Plans of Care.
  • Monitor and document changes in client condition.
  • Communicate with physicians, families, caregivers, and the care team.
  • Coordinate appropriate home care and additional services.
  • Complete assessments, clinical notes, and required documentation on time.
  • Provide client and caregiver education.
  • Ensure compliance with agency policies and applicable state and federal standards.

Qualifications

  • Active Florida RN License required.
  • Minimum 1 year of RN experience .
  • Home health/home care experience preferred.
  • Current BLS/CPR preferred.
  • Valid driver's license, reliable transportation, and auto insurance.
  • Strong assessment, documentation, and communication skills.
  • Work Location: In person / Client homes
  • Pay: $40.00 - $45.00 per hour
  • Work Location: In person
