About this role
LilyCare Hospice is a community-based hospice serving patients and families across eastern New Jersey. Hiring a full-time Hospice Registered Nurse Case Manager to join the clinical team and help shape hospice care delivery from the ground up. Responsibilities include serving as the Hospice RN Case Manager for a caseload of hospice patients in private homes and facilities, providing skilled pain and symptom management, participating in IDG meetings, educating and supporting patients, families, and caregivers, and completing timely EMR documentation.
Pays $90,000 - $105,000/year based on experience. Full-time, Monday–Friday day schedule with rotating on-call assignment. Territory based out of Florham Park, NJ.
