We are seeking a compassionate and dedicated Direct Support Professional, Licensed Vocational Nurse (LVN), or Registered Nurse (RN) to join our team in providing exceptional care and support to individuals with diverse needs. - Provide direct support and personal care assistance to individuals with developmental disabilities, neurodevelopmental disorders, or age-related conditions such as dementia and Alzheimer's care.
- Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods like Hoyer lifts.
- Monitor patient health by taking vital signs, observing for changes in condition, and accurately documenting findings in care plans while adhering to HIPAA regulations.
- Administer medications safely following prescribed instructions and document medication administration thoroughly.
- Support residents with meal preparation, feeding assistance, and maintaining a clean environment through cleaning duties aligned with health standards.
- Engage in patient observation and communication to ensure safety, comfort, and well-being; respond promptly to emergencies with first aid or CPR when necessary.
- Facilitate community outings or personal transportation assistance while respecting residents’ rights and promoting independence.
*Qualifications*
- Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred.
- Knowledge of patient monitoring techniques, care plans development, medication administration, and HIPAA compliance.
- Experience working with individuals with autism, intellectual disabilities, dementia, or neurodevelopmental disorders is advantageous.
- Certification as an LVN or RN is required; valid state licensure must be maintained throughout employment.
- Demonstrated ability to perform heavy lifting safely and use patient transfer equipment such as Hoyer lifts.
- Strong skills in patient observation, vital signs measurement, first aid/CPR certification, and personal transportation assistance.
- Excellent communication skills for effective client interaction and teamwork within a caregiving environment.
- Ability to handle manual tasks including assisting with ADLs and maintaining a clean living space while supporting resident’s rights and dignity.
