Unify Home Care is a locally owned, growing home care company seeking compassionate, dependable Licensed Practical Nurses (LPNs) to join our team in Jackson, Wyoming.

We provide personalized, one-on-one nursing care in patients' homes, allowing our nurses to build meaningful relationships while making a difference in their patients' everyday lives.

We're looking for nurses who enjoy providing hands-on patient care, value meaningful relationships, and appreciate a supportive, team-oriented environment.

Available Schedules & Pay

We are currently hiring for the following schedules

Weekday LPN – $34/hour

• Monday, Tuesday & Wednesday
• 8:30 AM – 9:00 PM

Weekend LPN – $36/hour

• Friday, Saturday & Sunday
• 8:30 AM – 9:00 PM
• Includes a $2/hour weekend differential

Both schedules offer

• Three scheduled shifts per week (37.5 scheduled hours)
• Four days off each week
• Consistent scheduling with established patients
• Hourly pay, not per patient visit
• No overnight shifts
• Holiday shifts not required

Responsibilities

• Provide individualized nursing care in patients' private residences within the LPN scope of practice.
• Monitor vital signs, patient conditions, and changes in health status.
• Administer prescribed medications and treatments as permitted by applicable nursing regulations.
• Assist with activities of daily living, personal care, mobility, and other daily needs.
• Recognize and promptly report changes in patient conditions.
• Communicate with patients, families, healthcare providers, and the care team.
• Complete accurate and timely nursing documentation.
• Follow established care plans, physician orders, and agency policies under appropriate clinical direction.
• Promote patient safety, comfort, dignity, and independence.

Qualifications

• Active LPN license valid for practice in Wyoming.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong practical nursing skills and attention to detail.
• Ability to provide care independently within the LPN scope of practice and seek guidance when appropriate.
• Dependable, compassionate, and professional.
• Excellent communication and organizational skills.
• Ability to successfully complete required background screening and drug testing.

Home care experience is preferred but not required. We welcome LPNs from a variety of nursing backgrounds who are passionate about providing quality patient care.
