Valid Registered Nurse (RN) license in good standing in the applicable state(s), with eligibility for multistate practice if needed. Experience in telehealth, community health, case management, or ambulatory care, preferably serving diverse or underserved populations. Strong clinical assessment and critical-thinking abilities, including triage, care planning, and evidence-based decision-making. Proficiency with electronic health records (EHR), telehealth platforms, and basic computer applications for documentation and communication. Excellent written and verbal communication skills, with the ability to provide clear, empathetic health education remotely. Demonstrated ability to work independently in a remote setting, manage time effectively, and maintain professional boundaries. Knowledge of HIPAA and other relevant privacy, safety, and regulatory requirements, and commitment to ethical practice. Associate or Bachelor’s degree in Nursing; additional certifications in community health, case management, or telehealth are a plus.
