About this role
We are seeking a dedicated and compassionate DD Waiver Registered Nurse (RN) to join our dynamic healthcare team. In this vital role, you will provide specialized nursing care to individuals enrolled in the Developmental Disabilities (DD) Waiver program, ensuring they receive comprehensive, person-centered support that promotes independence and quality of life. You will collaborate closely with healthcare professionals, caregivers, and families to develop and implement tailored care plans, monitor health status, and facilitate access to necessary services.

- Develop, implement, and evaluate personalized care plans aligned with each client’s goals and medical requirements.
- Manage case documentation using Therap), ensuring accuracy and compliance with HIPAA regulations.
- Coordinate discharge planning and post-acute care services to ensure seamless transitions across healthcare settings.
- Collaborate effectively with multidisciplinary teams—including physicians, therapists, social workers, and behavioral health specialists—to optimize patient outcomes.
- Provide clinical case management by overseeing medical interventions, infectious disease care, and managing complex health conditions.
- Facilitate intake assessments for new clients.
Qualifications
- Valid RN license with current registration in the state of NM.
- Knowledge of health regulation policies including HIPAA compliance and medical terminology.
- Proven ability to perform patient assessments accurately and manage complex clinical cases effectively.
- Strong communication skills for client interaction and collaboration with healthcare professionals.
- Familiarity with discharge planning, hospice care, and outpatient clinic operations is a plus.
- Ability to handle diverse populations.
