Responsibilities

  • Administer prescribed medications and document according to policy and MAR
  • Monitor residents, assess vital signs, and report changes to the supervising RN
  • Provide direct patient care to support activities of daily living (ADLs), mobility, hygiene, and feeding
  • Assist with wound care and dressing changes as ordered
  • Collaborate with the interdisciplinary care team to implement and update care plans
  • Maintain accurate documentation and participate in shift handoffs to ensure continuity of care

Requirements

  • Active LPN license in the state of New Mexico in good standing (or eligible for NM licensure)
  • Completion of an approved Practical Nursing program
  • Prior experience in a long-term care or skilled nursing facility setting preferred
  • Ability to work the listed shift hours (07:00–19:30) on a local assignment
  • Strong communication and teamwork skills; proficient in basic documentation
  • Must pass background check and drug screen as required and comply with facility policies
