Heartify In Home Care is seeking a dependable and experienced Licensed Practical Nurse (LPN) to provide skilled nursing services for an adult male client in Mableton, Georgia 30126, flexible schedule 1-2 time weekly. Applicants must have hands-on experience with tracheostomy care, feeding tubes and wound care.
Skilled Nursing Responsibilities
Provide tracheostomy care and monitor respiratory status
Monitor oxygen use and oxygen saturation
Assess and manage respiratory secretions and provide suctioning as ordered
Administer and monitor prescribed tube feedings through the client’s feeding tube
Provide feeding-tube site care and monitor for complications
Perform wound assessments, dressing changes, and wound care according to the physician’s orders
Provide medication teaching and monitor the client’s response to new medications
Monitor vital signs and changes in the client’s condition
Educate the client and family regarding the plan of care, safety precautions, and infection prevention
Communicate concerns or significant changes to the physician and clinical supervisor
Complete accurate and timely visit documentation
Qualifications
Active and unrestricted Georgia LPN license
Current CPR/BLS certification
Experience with tracheostomy care, suctioning, oxygen monitoring, feeding tubes, and wound care
Home health or complex-care experience preferred
Strong assessment, communication, and documentation skills
Valid driver’s license and reliable transportation
Must complete all required background and employment screenings
