Licensed Practical Nurse Needed (Flexible Schedule) 1-2 weekly visit

*Skilled Nursing Responsibilities*

• Provide tracheostomy care and monitor respiratory status
• Monitor oxygen use and oxygen saturation
• Assess and manage respiratory secretions and provide suctioning as ordered
• Administer and monitor prescribed tube feedings through the client’s feeding tube
• Provide feeding-tube site care and monitor for complications
• Perform wound assessments, dressing changes, and wound care according to the physician’s orders
• Provide medication teaching and monitor the client’s response to new medications
• Monitor vital signs and changes in the client’s condition
• Educate the client and family regarding the plan of care, safety precautions, and infection prevention
• Communicate concerns or significant changes to the physician and clinical supervisor
• Complete accurate and timely visit documentation

*Qualifications*

• Active and unrestricted Georgia LPN license
• Current CPR/BLS certification
• Experience with tracheostomy care, suctioning, oxygen monitoring, feeding tubes, and wound care
• Home health or complex-care experience preferred
• Strong assessment, communication, and documentation skills
• Valid driver’s license and reliable transportation
• Must complete all required background and employment screenings

• Immediate availability preferred
