Registered Nurse - RN Case Manager

We are seeking RN-Case Managers to provide in-home care for former Nuclear Weapons Workers with chronic or terminal illnesses resulting from exposure to toxic substances. Key Responsibilities Include Conducting weekly in-home nursing visits lasting 1-2 hours, performing physical assessments, and managing medications Ensuring home safety with medical equipment and supplies Overseeing skilled and non-skilled caregivers Collaborating with team members to improve services and address client needs Participating in care planning, documentation review, and quality improvement activities Supporting staffing, training, and clinical supervision Engaging in community outreach and supporting agency initiatives Qualifications Include Graduate of an approved nursing program, BSN preferred Current RN license in practice state(s) Minimum two years nursing experience, including one in home care CPR certification; supervisory or case management experience preferred Skills Needed Strong clinical assessment and care coordination skills Ability to work independently and as part of a team Excellent communication and documentation abilities Commitment to providing high-quality patient care This Role Offers Flexible schedule and work/life balance Competitive hourly rate of $47 plus • shift differentials Full benefits including medical, dental, vision, 401(k), paid time off, and voluntary benefits
