MDS Coordinator (RN)

Under the direction of the Director of Nursing, the Resident Assessment Coordinator (RAC) leads an interdisciplinary team using a core set of screening, clinical, and functional status elements that serve as the foundation of a comprehensive resident assessment. The RAC utilizes this assessment process to ensure each resident's plan of care reflects their individual needs, goals, and preferences.

The ideal candidate is a nurse with strong assessment and documentation skills who is detail-oriented, organized, and able to think critically. This individual enjoys collaborating with interdisciplinary teams and helping ensure residents receive high-quality, person-centered care.

The Details

  • Coordinate the interdisciplinary assessment process for assigned residents.
  • Coordinate the development and completion of Comprehensive Care Plans in accordance with company, state, and federal guidelines.
  • Monitor residents to ensure Care Plans are followed and updated as needed.
  • Identify and communicate variances to the Director of Nursing and Care Plan Team.
  • Collaborate with nursing, therapy, dietary, social services, and other departments to support resident care and outcomes.
  • Ensure assessment and documentation requirements are completed accurately and timely.

Qualifications

  • Current Registered Nurse (RN) license in good standing
  • Knowledge of nursing standards of practice and current clinical principles
  • Strong assessment, documentation, and organizational skills
  • Excellent analytical and critical thinking abilities
  • Computer proficiency and experience with electronic medical records and documentation systems
  • Previous MDS experience preferred but not required; we are willing to train the right candidate
