We are seeking a *Nurse Practitioner (NP)* who is passionate about prevention, chronic disease management, and the future of primary care.

Responsibilities
- Provide comprehensive adult primary care (18+)
- Manage acute and chronic conditions (e.g., HTN, DM, HLD)
- Perform preventive care, screenings, and wellness visits
- Incorporate lifestyle medicine into care plans
- Collaborate with care team to optimize patient outcomes
- Utilize care management programs (CCM/RPM) appropriately
Qualifications
- Licensed NP in Delaware (or eligible)
- Bilingual skills (Chinese or Spanish) preferred to support our diverse patient population.
