Hospice RN Case Manager Part-time Per Diem

Overview- Faith based locally owned hospice company expanding to Savannah area looking for compassionate RN to help serve Coastal Georgia patient, families and community.

As a part-time, per diem Hospice RN Case Manager, you’ll coordinate compassionate, individualized care for patients and support for their families. You’ll assess patient needs, guide care planning, and collaborate with healthcare professionals to help patients receive appropriate care across settings.

Duties

• Complete patient assessments, review vital signs and diagnostic information, and document care in electronic health records (EHR).

• Coordinate hospice care, intake, clinical case management, and communication with patients, families, and care teams.

• Maintain accurate medical records and follow Health Insurance Portability and Accountability Act (HIPAA) requirements and applicable health regulation policies.

• Collaborate with healthcare professionals to support continuity of care and appropriate utilization management.

Skills

• Required: Registered Nurse qualification and nursing experience; patient care, patient assessment, client communication, medical terminology, physiology, and EHR management skills.

• Preferred: Hospice care, home health, case management, or experience managing patients as a nurse case manager.

• Relevant experience may include acute, critical, intensive care unit (ICU), medical-surgical, inpatient, outpatient, clinic, hospital, post-acute, primary care, geriatrics, pediatrics, behavioral health, infectious disease, emergency room, emergency medicine, hospital medicine, or operating room (OR) care.

Pay: $45.00 - $50.00 per hour Expected hours: 15.0 – 20.0 per week
