RN/LPN- FULL TIME or PART TIME shifts available 11-7 & 3-11

• Perform ongoing *resident assessments*, including vital signs, pain assessments, and monitoring for changes in condition
• Administer medications and treatments accurately in accordance with physician orders and facility protocols
• Provide wound care, skin assessments, and monitor for pressure injuries and other skin impairments
• Complete timely and accurate documentation in *electronic medical records (EMR)* systems such as PointClickCare (PCC)
• Communicate changes in condition to physicians and obtain/implement new orders as needed
• Ensure all labs, diagnostics, and abnormal results are reported and followed up on promptly
• Supervise and delegate tasks to CNAs and ensure completion of assigned care
• Assist with *admissions, discharges, and transfers*, including medication reconciliation and required documentation
• Maintain infection control standards and ensure compliance with safety protocols
• Monitor and complete required facility logs (e.g., crash cart checks, glucometer logs, refrigerator logs)
• Educate residents, families, and staff on care plans, safety, and disease management

*Skills*

• Experience with EMR/EHR systems such as PointClickCare preferred
• Knowledge of long-term care regulations and standards of practice
• Ability to manage multiple residents in a fast-paced environment
• Skilled in medication administration, wound care, and basic nursing procedures
• Experience working with geriatric populations, including dementia care
• Strong communication and teamwork abilities
• Ability to prioritize tasks and respond effectively to emergencies

Pay: $27.00 - $40.00 per hour
