Join our dynamic healthcare team as a *Registered Nurse (RN) or Licensed Practical Nurse (LPN)* in a Skilled Nursing Facility. In this role, you will deliver hands-on nursing care to a geriatric population, ensuring safety, comfort, and optimal health outcomes. You will work closely with an interdisciplinary team to manage chronic conditions, respond to acute changes, and support residents through all stages of care. - Perform ongoing *resident assessments*, including vital signs, pain assessments, and monitoring for changes in condition
- Administer medications and treatments accurately in accordance with physician orders and facility protocols
- Provide wound care, skin assessments, and monitor for pressure injuries and other skin impairments
- Complete timely and accurate documentation in *electronic medical records (EMR)* systems such as PointClickCare (PCC)
- Communicate changes in condition to physicians and obtain/implement new orders as needed
- Ensure all labs, diagnostics, and abnormal results are reported and followed up on promptly
- Supervise and delegate tasks to CNAs and ensure completion of assigned care
- Assist with *admissions, discharges, and transfers*, including medication reconciliation and required documentation
- Maintain infection control standards and ensure compliance with safety protocols
- Monitor and complete required facility logs (e.g., crash cart checks, glucometer logs, refrigerator logs)
- Educate residents, families, and staff on care plans, safety, and disease management
*Skills*
- Experience with EMR/EHR systems such as PointClickCare preferred
- Knowledge of long-term care regulations and standards of practice
- Ability to manage multiple residents in a fast-paced environment
- Skilled in medication administration, wound care, and basic nursing procedures
- Experience working with geriatric populations, including dementia care
- Strong communication and teamwork abilities
- Ability to prioritize tasks and respond effectively to emergencies
Pay: $27.00 - $40.00 per hour
