REGISTERED NURSE (RN) – ADMISSIONS | CLINICAL LEADER$40/HR WEEKENDS + MILEAGE | $35/HR WEEKDAYS + MILEAGE

Elite Professionals Home Care is looking for an experienced Home Health RN to join our team as an Admissions/Clinical Leader. If you’re confident in your clinical skills, passionate about patient care, and ready to make a difference from the very first visit, we want you!
The schedule & pay
- $40/hour + mileage – Saturday & Sunday
- Every other weekend minimum
- Optional weekday hours: Monday/Thursday OR Tuesday/Friday
- $35/hour + mileage for weekday visits
- Typical hours: 8:00 AM–5:00 PM
What you’ll do
- Complete OASIS admissions, assessments, recertifications, resumptions, and discharges
- Develop individualized Plans of Care
- Collaborate with physicians, therapists, aides, patients, and families
- Provide clinical guidance and support to nursing staff
- Perform skilled nursing assessments and patient visits
- Utilize Homecare Homebase (HCHB) EMR

What we’re looking for
✔ Active Nebraska RN license
✔ Home health/home care experience required
✔ Strong clinical assessment and documentation skills
✔ Compassionate, confident, and dependable
✔ Wound care experience preferred
✔ Valid driver’s license, reliable transportation, and appropriate insurance
