Job Summary

Green Acres Healthcare Services is seeking a dedicated and experienced Licensed Practical Nurse (LPN) for a temporary, travel-based assignment. This position involves providing private duty care in the home setting, with a strong preference for candidates experienced in tracheostomy care, mouth/oral suctioning, and ventilator-dependent patients.

Key Responsibilities

Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, and toileting
Assist with mobility, transfers, and positioning
Take and record vital signs (temperature, pulse, respiration, blood pressure)
Assist with feeding and monitor nutritional/fluid intake as directed
Perform light housekeeping related to client care (changing linens, tidying care areas)
Maintain accurate, timely documentation of care provided
Follow the individualized care plan and all agency policies and procedures
Maintain client confidentiality in accordance with HIPAA
Perform tracheostomy care and mouth/oral suctioning as needed
Monitor and manage ventilator-dependent patients, including troubleshooting equipment and responding to alarms
Respond promptly to changes in patient condition and escalate care needs appropriately
Maintain accurate clinical documentation in compliance with HFRD regulations
Communicate effectively with family members, physicians, and the care team
Travel to assignment location(s) as required for this position

Qualifications

Active, unencumbered Georgia LPN license
Current CPR/BLS certification
Experience with tracheostomy care and suctioning preferred
Experience with ventilator-dependent patients strongly preferred
Willingness and ability to travel for this assignment
Reliable transportation
Strong communication skills and a caring, patient demeanor
Ability to pass background check and drug screening
Current TB test/PPD and up-to-date immunizations

Schedule

Temporary assignment; shift schedule and duration to be discussed with candidate
