*Job Summary* Green Acres Healthcare Services is seeking a dedicated and experienced Licensed Practical Nurse (LPN) for a temporary, travel-based assignment. This position involves providing private duty care in the home setting, with a strong preference for candidates experienced in tracheostomy care, mouth/oral suctioning, and ventilator-dependent patients.

*Key Responsibilities*

  • Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, and toileting
  • Assist with mobility, transfers, and positioning
  • Take and record vital signs (temperature, pulse, respiration, blood pressure)
  • Assist with feeding and monitor nutritional/fluid intake as directed
  • Perform light housekeeping related to client care (changing linens, tidying care areas)
  • Maintain accurate, timely documentation of care provided
  • Follow the individualized care plan and all agency policies and procedures
  • Maintain client confidentiality in accordance with HIPAA
  • Perform tracheostomy care and mouth/oral suctioning as needed
  • Monitor and manage ventilator-dependent patients, including troubleshooting equipment and responding to alarms
  • Respond promptly to changes in patient condition and escalate care needs appropriately
  • Maintain accurate clinical documentation in compliance with HFRD regulations
  • Communicate effectively with family members, physicians, and the care team
  • Travel to assignment location(s) as required for this position

*Qualifications*

  • Active, unencumbered Georgia LPN license
  • Current CPR/BLS certification
  • Experience with tracheostomy care and suctioning preferred
  • Experience with ventilator-dependent patients strongly preferred
  • Willingness and ability to travel for this assignment
  • Reliable transportation
  • Strong communication skills and a caring, patient demeanor
  • Ability to pass background check and drug screening
  • Current TB test/PPD and up-to-date immunizations
