We are seeking a dedicated and compassionate Part-Time Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality care to one client at their home. Your expertise will support patient assessments, medication administration, and vital sign monitoring, seizures monitoring to ensuring comfort and safety for our patients. - Conduct comprehensive patient assessments, including vital signs and physical examinations, to inform care plans.
- Administer medications and G-tube feedings as ordered by physician.
- Maintain detailed medical records, ensuring compliance with HIPAA and confidentiality policies.
- Support patients with activities of daily living, including mobility assistance with Hoyer lifts and tube feeding management.
- Collaborate with interdisciplinary teams on discharge planning, case management, and patient education to promote optimal health outcomes.
- Respond promptly to urgent situations such as acute pain episodes or airway management needs in various clinical settings like ICU or emergency rooms.
*Experience*
- Valid LPN license with current state licensure required.
- Prior experience working in home care required.
- Demonstrated proficiency in EMR/EHR systems including Epic, Cerner, or Athenahealth.
- Experience with specialized care areas such as Alzheimer's/dementia care, memory care, or geriatrics preferred.
- Knowledge of documentation standards for accurate medical records management.
- Familiarity with infection control practices and sterile techniques in clinical procedures
