What You'll Do
Lead, supervise, and develop all nursing staff including RNs, LPNs, QMAs, and CNAs
Oversee nursing operations across all shifts and ensure consistent clinical standards facility-wide
Ensure compliance with CMS Conditions of Participation, state health department regulations, and facility policies
Drive quality outcomes including reduction of falls, pressure injuries, hospitalizations, and infections
Lead the facility's infection prevention and control program
Oversee the MDS process, care planning, and PDPM reimbursement accuracy in collaboration with the MDS Coordinator
Manage nursing department staffing, scheduling, and labor utilization
Recruit, hire, onboard, and retain high-quality nursing staff
Lead QAPI initiatives and present clinical data and improvement plans to administration
Prepare for and lead the clinical response to annual state surveys and Immediate Jeopardies
Serve as the clinical decision-maker and escalation point for complex resident care situations
Partner with the Medical Director, attending physicians, and specialists on resident care
Requirements
Current, unrestricted RN license in the state of Indiana required
Minimum 3 to 5 years of nursing leadership experience — SNF or long-term care experience strongly preferred
Strong working knowledge of CMS Conditions of Participation, survey process, and PDPM
Demonstrated ability to lead and develop a large, multi-shift nursing team
Experience managing MDS, care planning, and clinical quality metrics
Strong clinical assessment, critical thinking, and decision-making skills
Proven track record of survey preparedness and regulatory compliance
Current BLS/CPR certification required
