- Perform detailed patient assessments, including vital signs, physical examinations, and comprehensive health evaluations to determine individual care needs.
- Administer medications via IV infusion, injections, and other routes while adhering to aseptic techniques and safety protocols.
- Manage complex cases involving acute pain management, wound suturing, ostomy care, and ventilator support in the home setting.
- Utilize EMR (Electronic Medical Records) systems such as Epic or Cerner to document patient care accurately and efficiently.
- Coordinate discharge planning and collaborate with physicians on treatment plans involving botulinum toxin treatment, dialysis, or cardiac catheterization follow-up.
- Provide education on medication administration, disease management—including infectious disease care—and health coaching tailored to patients’ conditions.
- Respond promptly to triage calls and urgent situations, ensuring appropriate interventions for patients experiencing acute or chronic issues.
- Maintain compliance with HIPAA regulations and ensure all medical documentation is complete and accurate for legal and billing purposes.
*Experience*
- Proven experience in home health nursing or related outpatient settings with familiarity in medical-surgical, ICU, or emergency medicine environments.
- Hands-on experience with advanced clinical procedures such as IV insertion, catheterization (urinary or central lines), phlebotomy, and sterile processing techniques.
- Knowledge of ICD-10/ICD-9 coding systems for accurate documentation and billing processes.
- Critical care experience preferred, including ICU or trauma center exposure at Level I or Level II trauma centers.
- Demonstrated ability to work with individuals with disabilities or developmental delays across various settings like assisted living or memory care facilities.
- Strong understanding of laboratory testing procedures, specimen collection/processing, and diagnostic evaluation methods.
- Familiarity with EMR/EHR systems such as Athenahealth or eClinicalWorks to ensure seamless documentation of patient interactions.
