Responsibilities

• Administer prescribed medications and treatments per physician orders.
• Provide direct nursing care to residents, including monitoring for changes in condition.
• Document assessments, vitals, care plans, and interventions accurately in the charting system.
• Collaborate with the interdisciplinary team to coordinate resident care and ensure safety and quality standards.
• Assist with daily living activities, wound care, and routine nursing procedures as appropriate.
• Uphold regulatory compliance, safety, and infection control practices in a long-term care environment.

Requirements

• Active Licensed Practical Nurse license in New Hampshire, in good standing.
• Ability to perform nursing duties in a long-term care setting and follow physician orders.
• Availability for evening shifts with a 40-hour per week schedule.
• Strong communication and documentation skills; capable of working with an interdisciplinary team.
• Must be able to meet background check and health screening requirements as required by practice regulations.
