Getting Discharged Home Is One Thing. Staying Home Is Another.
The medications changed.
There are follow-up appointments to schedule. Therapy is involved. The family is trying to understand new instructions. The patient is weaker than they expected to be.
And suddenly, the discharge plan that looked straightforward on paper doesn't feel quite so simple.
That's where a great RN Case Manager can change the trajectory.
Home Health Care, Inc. is hiring an experienced Registered Nurse (RN) Case Manager to serve patients throughout Woodbury, Cottage Grove, Hastings, St. Paul Park, Landfall, and surrounding communities .

Be the Clinical Bridge Between Discharge and Recovery
A hospital can provide excellent care and a detailed discharge plan.
But the real test starts when the patient gets home.
Are they taking their medications correctly? Can they safely get around the house? Does the caregiver understand the plan? Are symptoms improving or getting worse? Is therapy progressing? Does the physician need an update?
As the RN Case Manager, you help connect what was supposed to happen with what's actually happening.
You'll assess, educate, coordinate, communicate, and intervene when necessary to keep recovery moving forward.

Sometimes the Biggest Win Is the Hospitalization That Never Happens
A patient gains five pounds unexpectedly and you recognize the concern.
A caregiver has been giving a medication incorrectly and you catch it.
A wound starts changing and you get the right people involved.
A patient doesn't understand which symptoms should trigger a phone call, so you teach them before there's an emergency.
Those moments may never show up as dramatic nursing stories.
But they're exactly why strong home health nursing matters.
You're helping patients recognize problems earlier, understand their conditions better, and have the clinical support necessary to stay safely at home.
Bring the Experience You've Already Built
We're interested in experienced RNs from Home Health, Med/Surg, Acute Care, ICU, Transitional Care, Skilled Nursing, Rehabilitation, Case Management, and other clinically complex settings .
If you've spent your career assessing patients, prioritizing clinical needs, educating families, communicating with providers, and recognizing when something isn't right, you've already developed many of the skills that matter in home health.

Help Make Sure Home Is Where They Stay.
Discharge isn't the finish line.
For many patients, it's the beginning of the hardest part of recovery.
