As a field RN, you will play a vital role in assessing patient needs, administering treatments, and ensuring optimal health outcomes through personalized home health services. *Duties*
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews.
- Administer medications via IV infusion, injections, and other routes following proper aseptic techniques.
- Perform specialized procedures such as wound care, ostomy care, catheterization, and tube feeding management.
- Manage complex cases involving acute pain, infectious diseases, or chronic conditions like memory care and various other chronic disease processes (HTN, Heart Failure, COPD, Diabetes, etc)
- Educate patients and families on disease management, medication administration, and health coaching strategies.
- Coordinate with interdisciplinary teams to develop and implement personalized care plans aligned with Medicare and State standards and HIPAA regulations.
- Document all patient interactions accurately using EMR/EHR systems; ensure compliance with medical documentation standards.
*Qualifications*
- Current RN licensure with active state registration.
- Critical care experience in ICU or emergency medicine settings preferred.
- Ability to perform venipuncture and laboratory specimen collection/processing.
- Excellent patient assessment skills with the ability to manage complex cases involving post-acute care needs.
- Knowledge of infection control practices and aseptic techniques essential for safe patient care.
- Experience in home & community care settings
- BLS certification required.
Job Type: Full-time Pay: From $78,000.00 per year
