About this role

We are seeking a highly motivated and organized MDS Coordinator to join our team at Ventura Services Florida in Miami, FL. As a key member of our healthcare team, you will play a critical role in ensuring the accurate and timely completion of Minimum Data Sets (MDS) for our residents. Responsibilities

Coordinate the collection and submission of accurate and complete MDS data in accordance with federal regulations and company policies

Develop and maintain relationships with residents, families, and healthcare team members to ensure a comprehensive understanding of resident needs and preferences

Conduct and document detailed assessments of resident needs and preferences, including social, emotional, and environmental factors

Collaborate with the interdisciplinary team to develop and implement comprehensive care plans that promote resident well-being and quality of life

Monitor and report on MDS data to identify trends and areas for improvement, and make recommendations for changes to the care plan

Participate in quality improvement initiatives and contribute to the development of policies and procedures related to MDS coordination

Maintain accurate and up-to-date records and Files, including MDS assessments, care plans, and other relevant documentation

Requirements

Minimum 1 years of experience in a healthcare setting, with a focus on gerontology and MDS coordination

Strong communication and interpersonal skills, with the ability to effectively interact with residents, families, and healthcare team members

Ability to work independently and as part of a team, with a focus on delivering high-quality care and excellent customer service

Proficiency in electronic medical records and MDS software systems
