Join Our Team at The Avenue at Strongsville

The Avenue at Strongsville, a post-hospital rehabilitation and skilled nursing care facility, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As a valued member of our healthcare community, you will have the opportunity to make a meaningful difference in the lives of our residents and patients.

About the Role

As an LPN at The Avenue at Strongsville, you will be responsible for providing exceptional nursing care to our residents and patients. This includes:

Assessing and documenting patient condition, including vital signs, medical history, and treatment plans

Administering medications and therapies as prescribed by our medical team

Developing and implementing individualized care plans to meet each patient's unique needs

Collaborating with our interdisciplinary care team to ensure seamless coordination of care

Educating patients and their families on their condition, treatment options, and disease management

Maintaining accurate and timely records of patient care and treatment
