Licensed Practical Nurse (LPN)
- Hours: Monday - Friday; Standard Business Hours & Occasional On-Call Rotation
- Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely
- Territory: ~30-Mile Radius (Mileage Reimbursed)
- Pay Rate: $28.00 - $31.00 per Hour

Responsibilities
Provide direct patient care under the supervision of a registered nurse. Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes. Implement the plan of care initiated by the registered nurse.
Provide accurate and timely documentation consistent with the plan of care. Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care. Participate in coordination of home health and hospice services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
Use and/or prepare equipment and supplies effectively and efficiently adhere to aseptic techniques. Participate in personal and professional growth and development. Perform other duties assigned by the registered nurse, including caring for home care patients as census allows.
Participate in weekend and holiday on-call rotation requirements.
Qualifications
Must be a graduate from an accredited practical nurse or vocational nursing program. Must hold a valid/active license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio. Prior hospice experience is highly preferred. Prior experience in community/home health and/or medical/surgical, preferred. 3 years of nursing experience, preferred. Comply with accepted professional standards and practices. Demonstrates good verbal and written communication, and organizational skills. Must be able to lift, position or transfer patients and lift supplies and equipment.
