Job Summary RN Liaison Responsible for ensuring that patients referred to Evergreen Home Care Services (EHCS) have a smooth transition to Home Care services in the care setting of their choice, educating the community, physicians and other health care providers about Hospice, Palliative Care, Home Health, and Behavioral Health services.

Responsibilities Responsible for growth of referral base and consistent improvement in community reputation as a leader of Home Care services. Assesses the appropriateness for Home Care Services for patients in hospitals, skilled nursing facilities, adult family homes and assisted living facilities. Interacts and engages daily with hospital staff, skilled nursing facility staff, physician office staff and the Home Care administrative and clinical staff.

Initiates and maintains collaborative relationships and clear communication between the EHCS management and referral sources. Participates in customer service measurements and functions, collecting analyzing and sharing information with EHCS management. Responsible for maintaining and growing existing and new relationships with referral sources
