Position Summary
The Registered Nurse (RN) Targeted Case Manager provides person-centered case management and care coordination services to individuals with intellectual, developmental, physical, and/or other disabilities and special needs. The RN works collaboratively with participants, families, caregivers, healthcare providers, community agencies, and other members of the interdisciplinary team to promote health, safety, independence, community integration, and access to necessary services.
The position primarily involves assessment, service planning, monitoring, advocacy, resource coordination, and documentation in accordance with applicable South Carolina Department of Health and Human Services (SCDHHS), Medicaid, program, and agency requirements.
Essential Duties and Responsibilities
Conduct comprehensive assessments and identify the participant's medical, functional, behavioral, social, environmental, and support needs.
Develop and assist with person-centered service plans based on the participant's assessed needs, preferences, strengths, and goals.
Coordinate Medicaid, healthcare, behavioral health, social, community, and other supportive services.
Monitor participants' health status, functional abilities, safety, living environment, and access to needed services.
Identify changes in condition or circumstances that may require reassessment, additional services, referrals, or modifications to the service plan.
Communicate and collaborate with physicians, nurses, social workers, therapists, direct-care providers, caregivers, families, and other members of the participant's support team.
Assist participants and families with locating appropriate community resources, medical services, transportation, equipment, supplies, and other supports.
Advocate for participants to ensure their needs, preferences, rights, and choices are appropriately represented.
Provide education and support to participants and caregivers regarding health conditions, medications, safety concerns, and available community resources within the RN's professional scope.
Complete required participant contacts, home visits, assessments, service plans, monitoring activities, and follow-up within established timeframes.
Maintain accurate, complete, timely, and confidential case documentation.
Recognize and report suspected abuse, neglect, exploitation, significant health concerns, and other safety issues according to applicable requirements.
Participate in interdisciplinary team meetings, case conferences, and care-planning activities as required.
Maintain compliance with HIPAA, Medicaid requirements, SCDHHS policies, agency policies, and applicable professional standards.
Perform other case-management and care-coordination duties consistent with the position and program requirements.
Minimum Qualifications
Current and unrestricted Registered Nurse (RN) license in South Carolina.
Graduate of an accredited nursing program.
Ability to work effectively with individuals with intellectual, developmental, physical, and/or other disabilities and special needs.
Strong assessment, documentation, communication, organization, and care-coordination skills.
Ability to work collaboratively with participants, families, healthcare professionals, social-service providers, and community organizations.
