Requirements
- Associate Degree in Nursing required; Bachelor of Science in Nursing preferred
- Completion from an accredited school of nursing
- Minimum 2 years nursing experience preferred
- 1-3 years experience in a Federally Qualified Health Center (FQHC) or primary care field required
- Current unrestricted Oregon RN license
- Current unrestricted Washington RN license required (or ability to obtain within 6 months of hire)
- Active BLS certification required
- National Provider Identifier (NPI) required upon hire
