About the Role

As a Pediatric Home Health LPN/RN with Suma Home Care, your primary responsibilities will include:

Providing skilled nursing care to pediatric patients in their homes, including wound care, medication management, and IV administration

Collaborating with patients, families, and healthcare teams to develop and implement individualized care plans

Conducting comprehensive assessments of patients' physical and emotional needs

Educating patients and families on self-care techniques, medication management, and disease management

Documenting patient information and communicating with healthcare teams in a timely and accurate manner

Working closely with caregivers and home health aides to ensure seamless transitions and continuity of care

Ideal Candidate

Pediatric home care experience is preferred, but not required. We will consider qualified candidates who are eager to learn and grow with our team.

Current LPN/RN license in the state of Pennsylvania with a strong understanding of pediatric nursing practices and standards.

Excellent communication and interpersonal skills to build strong relationships with patients, families, and healthcare teams.

Ability to work independently in a fast-paced environment while prioritizing patient safety and satisfaction.

Basic computer skills, including proficiency in electronic medical records and communication systems.
