About this role
Role: Registered Nurse providing home-based nursing care in compliance with laws, regulations, and agency policies.
Responsibilities
- Deliver high-quality clinical services within scope of practice and infection control standards
- Coordinate care with healthcare team from admission to discharge
- Conduct clinical nursing assessments per program and payer requirements
- Verify patient eligibility and medical necessity for services
- Develop and revise individualized care plans with community providers
- Ensure care plan appropriateness and obtain physician approval for revisions
