Description: At Konza Prairie Community Health Center, we are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our integrated care team. This is a full-time opportunity to provide patient-centered care in a collaborative, mission-driven environment/ As a vital member of our care team in a Federally Qualified Health Center (FQHC), you will support the delivery of outpatient primary care alongside providers, behavioral health professionals, and fellow clinical staff. Our model emphasizes whole-person, evidence-based care—giving you the time and support to focus on quality, build meaningful patient relationships, and make a lasting impact in the communities we serve. What You’ll Do Perform patient assessments, triage, and document medical histories Collaborate with providers and care team members to deliver high-quality primary care Accurately maintain patient records in the electronic medical record (EMR) Administer medications, immunizations, and treatments to patients of all ages Assist with procedures and coordinate care, including follow-up and referrals Provide patient education and triage phone calls to support ongoing care Why Join Konza?
