Are you a compassionate Certified Nursing Assistant who enjoys providing one-on-one care in the home setting? We are seeking dedicated CNAs to join our growing team serving older adults throughout the Loveland area. This role offers the opportunity to make a meaningful impact by helping participants remain safe, independent, and comfortable in their homes.

Schedule

  • Wednesday-Saturday 7am-530pm (4x10)

Position Summary

The In Home Services CNA is responsible for providing routine personal care, restorative care, and supportive services to participants in their homes. This role works closely with the interdisciplinary care team to support participants' health, safety, and quality of life.

Responsibilities

  • Assist participants with activities of daily living including bathing, dressing, grooming, oral care, toileting, feeding, and personal hygiene.
  • Follow established care plans and document services provided accurately and timely.
  • Monitor and report changes in participant condition, appetite, skin integrity, functional abilities, and overall well-being.
  • Obtain and record vital signs and weights as assigned.
  • Assist with ambulation, transfers, positioning, skin care, catheter care, oxygen support, and other restorative care activities.
  • Provide assistance during meals and feeding when necessary.
  • Communicate participant concerns, incidents, accidents, or unusual occurrences to the clinical supervisor promptly.
  • Participate in care planning and collaborate with the interdisciplinary team.
  • Perform home care services and household tasks essential to participant health and safety.
  • Complete visit documentation and records in accordance with agency policies.
  • Attend required in-services, training programs, and quality improvement activities.
  • Perform additional duties as assigned.

Required Qualifications

  • Current and valid Colorado Certified Nursing Assistant (CNA) certification.
  • Current BLS and First Aid certification prior to hire (American Heart Association or American Red Cross accepted).
  • Valid driver's license, reliable personal transportation, and current auto insurance.
  • Minimum of one (1) year of experience working with frail, elderly, or geriatric populations.

Preferred Qualifications

  • Bilingual skills are highly preferred.
  • Previous home health, home care, hospice, senior living, long-term care, or PACE experience.
