Excelcare (LOCATION) is seeking a dedicated Licensed Practical Nurse (LPN) who provides direct patient care under RN and physician supervision. Responsibilities include administering medications, monitoring vital signs, assisting with daily care needs, documenting clinical information, and supporting residents and families. LPNs help maintain safety, comfort, and high quality care while collaborating with the interdisciplinary team and following all regulatory and infection control standards.

Responsibilities of a Licensed Practical Nurse (LPN)

  • Deliver hands-on nursing care in accordance with care plans and facility protocols
  • Administer medications and treatments as prescribed
  • Monitor and report changes in residents’ health and emotional status
  • Collaborate with nurses, physicians, and care team members
  • Maintain accurate documentation and ensure compliance with safety standards
  • Support a clean, safe, and respectful environment for residents

Qualifications of Licensed Practical Nurse (LPN)

Must have a Licensed Practical Nursing license in the state. Proficient in medication administration, vital signs, treatments, and documentation. Prior long term care or skilled nursing experience preferred. Understanding of state regulations, infection control, and resident rights.
