Position: Licensed Practical Nurse - LPN Location: Portage, WI 53963 (If this location does not work, we have other openings in Waupun, Oshkosh, and Fond du Lac, WI) Duration: 26 weeks (Possible extension) Pay rate: $38/hr. Setting: Corrections Schedule: 6 am - 6:30 pm | 36 hours weekly (includes every other weekend and holidays) Key Responsibilities Administer medications and treatments as prescribed by healthcare providers. Perform patient assessments and document findings accurately and timely. Monitor and evaluate patient health status, reporting significant changes to the RN or provider. Provide wound care, chronic disease management, and routine nursing interventions. Assist with intake screenings, health assessments, and discharge planning. Maintain accurate medical records in accordance with facility policies and regulatory requirements. Respond appropriately to medical emergencies and urgent healthcare needs. Educate patients on health conditions, medications, and treatment plans. Collaborate with interdisciplinary healthcare and correctional teams to ensure continuity of care. Adhere to infection control, safety, and security procedures at all times. Qualifications Current, unrestricted Licensed Practical Nurse (LPN) license. Current CPR/BLS certification. At least 1 year of LPN experience is required Previous correctional, behavioral health, emergency room, medical-surgical, or long-term care experience is preferred #Workwolf
