The Visiting Nurse Association of the Treasure Coast is seeking an Admissions RN to work with Hospice. Join our compassionate, dedicated Admissions team in bringing comfort, caring and relief to patients and their families. Must be currently licensed as an RN in the state of Florida. Hospice experience is strongly preferred but applicants with two years recent acute care experience will be considered.
Responsibilities
- Identify patient/family needs and provide admission services in accordance with the federal and state guidelines.
- Assumes primary responsibility for initial patient/family education related to hospice election, services and hospice coverage as it relates to the hospice diagnosis. Initiate communication with attending physicians, Interdisciplinary group (IDG) and other agencies, needed to coordinate optimal care and use of resources for the patient/family and their immediate needs.
- Maintain regular communication with the RN Patient Care Manager and the hospice team coordinator/scheduler to review caseload.
- Inform the RN Patient Care Manager of unusual or potentially problematic patient/family issues.
- In a timely manner obtain data on physical, psychological, social and spiritual factors that substantiate hospice eligibility.
- Obtain up-to-date medical records, seek input from other team members regarding the patient plan of care, and obtain additional knowledge that supports hospice eligibility and end of life prognosis.
- Meet regularly with Hospice nursing staff to review problems or unique issues, share professional support and exchange feedback aimed toward enhancing professional growth.
- Share in providing 24 hour, seven-day week coverage to patient/families.
- May be responsible to take after hours or weekend on call.
- Will work one summer and one winter holiday.
- Assume role of associate nurse during on-call time or to fill patient/family needs during the absence of other primary nurses.\
- Provide appropriate support at time of death and period of bereavement if indicated.
- Participate in VNA Hospice orientation and in-service training programs for professional staff.
- Participate in agency and community programs as requested, to promote professional growth and understanding of Hospice care.
Qualifications
- Current RN license in the State of Florida
- Minimum of 1 year experience as an RN in Hospice Care
- Extensive knowledge of Hospice Conditions of Participation with Medicare preferred
