Responsibilities

*Adheres to the LPN scope of practice *Administer prescribed medications and treatments, as ordered by provider/Standard of Care Protocol. *Provides patient care in the ambulatory care setting according to established standards in collaboration with Providers, Registered Nurses and Medical Assistants as members of the care team. *Provides primary support to the provider by proactively anticipating the provider needs, preparing the patient for the visit, maintaining efficient patient flow, and assisting with treatments and procedures. *Participates on interdisciplinary team to meet patient care and clinic needs, working cooperatively with others to accomplish required work in a timely and efficient manner. *Assists providers with procedures. *Assists with patient education, within scope of practice by Standard of Care-Education Protocol. *Assists with visit planning, within Health Maintenance Guidelines. *Assist with management, in collaboration with Clinical Manager. *Documents in electronic medical record within scope of practice. *Assists in screening of patients, in collaboration with Care Manager. *Assists Certified Medical Assistants with patient preparation/rooming patients, as directed by visit plan. *Applies Compassionate Connected Care principles and incorporates into care interactions *Interacts effectively with customers, providing instructions and information, both written and verbal, as appropriate. *Maintains established practice policies and procedures, objectives, quality program, safety, environment, and infection control standards. *Seeks help, communicates, and cooperates with Medical Provider and team appropriately. *Meets assigned productivity targets *Performs other functions as required.
