Executive Home Care of Augusta is a veteran-owned, concierge-level home care agency seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our team. We help seniors across the CSRA maintain their independence and remain safely at home — and we do it through personalized, unhurried, relationship-based care, not a rotating caseload.

What You'll Do
As an LPN, you'll serve as a clinical resource for our clients, caregivers, and leadership team by:
- Conducting nursing and wellness visits
- Monitoring client health status and documenting observations
- Performing medication reconciliation and medication education
- Assisting with chronic disease education and wellness coaching
- Communicating changes in client condition to physicians and family members
- Supporting care plan development and updates
- Training and mentoring caregivers on client-specific care needs
- Assisting with hospital-to-home transitions
- Maintaining accurate clinical documentation
- Collaborating with our Care Managers to ensure exceptional client outcomes
- Qualifications
Current Georgia LPN license in good standing Minimum one year of nursing experience (home care, geriatrics, hospice, assisted living, rehabilitation, or skilled nursing preferred) Strong assessment and communication skills Compassionate, patient-centered approach Reliable transportation and valid driver's license Ability to pass background screening and drug screening Current CPR certification Preferred Experience
